EPP Affiliate Confirmation Form

Please complete this form in its entirety.  You have been identified as an "Affiliate Exhibitor" under SCI Convention Policies, specifically SCI's Exhibitor Partnership Placement Program ("EPP Program"), which will allow the Main Exhibitor, also identified in this document, to select a booth location for the Affiliate Exhibitor due to certain affiliation relationships the two organizations may have. 

The purpose of this AEPPC Form is to confirm that Exhibitor identified as an Affiliate Organization is (i) aware of the EPP Form submitted including it as an Affiliate Organization to have it co-located with the Main Exhibitor, (ii) to give up its right to select its own booth location and allow the Main Exhibitor to select the booth location contiguous or as close thereto as possible, space at the Convention, and (iii)Certain information is requested for the organization requesting status as the "Main Exhibitor" to determine if that organization meets each of the requirements.  There is then information requested about each organization identified as an "Affiliate Exhibitor" to determine if the organization meets the requirements to be listed as such.

Terms
By signing below, I certify and confirm each of the following on behalf of the identified Affiliate Exhibitor:

I am authorized to sign on behalf of the organization identified as the Affiliate Exhibitor, above, and the above information concerning that organization is true and correct to the best of my knowledge and belief;

Affiliate Exhibitor is affiliated with the "Main Exhibitor" identified above in the manner described and supported with additional documentation submitted herewith;

Affiliate Exhibitor consents to being named as such and to allowing SCI to follow instructions from Main Exhibitor in the selection and placement of a booth space for Affiliate Exhibitor for the upcoming Convention, with such booth likely being located together (e.g., contiguous to, or as close thereto as practicable) with that of the Main Exhibitor, at the upcoming SCI Convention; and

Affiliate Exhibitor recognizes its obligations to (i) execute an Exhibitor Contract directly with SCI, (2) adhere to all SCI Convention Policies; (3) pay directly to SCI all required booth and other fees to participate in the SCI Convention; (4) make an independent donation to SCI as required by the Convention Contract, and (5) pay the additional $1,500 EPP fee to SCI if the EPP is implemented.
